Faculty Information Forms and Conflict of Interest
Full-time faculty members are required to complete annual reporting and disclosure requirements established by Columbia University and the School of International and Public Affairs.
These requirements support faculty appointments, governance activities, accreditation and reporting obligations, conflict of interest review, and other administrative processes.
SIPA Faculty Information Form (F-I-F)
Faculty are required to complete the annual Faculty Information Form and update their information as necessary. The information collected supports faculty records, publications, biographies, governance activities, and institutional reporting.
Information regarding annual reporting deadlines and procedures is distributed by the Office of Academic Affairs.
SIPA Conflict of Interest Policy
The SIPA conflict of interest policy requires all full-time faculty members to report on their Faculty Information Form any outside professional activities that might reasonably be perceived to pose a conflict of interest.
Faculty who engage in outside professional activities are also required to include an "Outside Professional Activities" section on their curriculum vitae listing such activities during the preceding five years and to make their curriculum vitae available online.
In addition, all faculty members must complete the annual Conflict of Interest Disclosure through the University Rascal System.
