Forms
The Student Affairs Office offers forms to communicate routine student requests. They are available on the carousel at the entrance to the Student Affairs Office, room 610. Please return completed forms to the person or office indicated on the form.
Form #1: Application for Degree or Certificate
This form is required for all graduating students. Deadlines and conferral dates are August 1 for October graduation; November 1 for February graduation; and December 1 for May graduation.
Form #2: Application for Advanced Standing (PDF)
This form is required to request credit for relevant graduate-level courses taken for credit after completion of the undergraduate degree.
It is also necessary in order to request credit for relevant graduate-level courses taken during undergraduate studies. This is granted only if courses were taken as electives beyond degree requirements.
Courses submitted for advanced standing credit must have a minimum earned grade of B. Language courses are not eligible for advanced standing.
To petition for advanced standing credits, please submit course descriptions and an official transcript.
Form #3: Application for Individual Reading and Research Course (PDF)
This form is needed to apply for an independent study with a faculty adviser.
The dean's approval must be obtained before registering for an independent study course.
It must be submitted with a completed add/drop form.
Form #4: Leave Of Absence (LOA) Request (PDF)
This is for students wishing to take a leave of absence for a limited period of time; usually one semester.
Students must obtain the dean's approval before taking a leave of absence.
Form #5: Application for Incomplete (PDF)
To request an incomplete in a course, complete and submit an Application for Incomplete form with a pedagogic reason for the incomplete, the professor's signature, and the signature of an assistant dean.
Form #6: Student Letter Request (PDF)
This is for verification of enrollment and academic standing for employment, internships, and visa extensions.
An original letter will either be placed in the student pickup box at the Student Affairs Office reception or sent to the address that you provide.
Please allow two working days for your request to be processed.
Form #7: Room Reservation Request
This is used for any room reservations in the building except for the 15th floor and seminar rooms listed on the form. Please allow five working days for this request. Reserve rooms early. Flexibility on dates and locations may be necessary.

If you need AV equipment, you will need to submit the Online AV form after your request has been approived. Please note that AV support is only avaiable Monday-Friday from 9am till 8:30pm.

Form #8: Audiovisual Equipment Reservations
All reservations must be made a week in advance. Reservation requests can be submitted online at the Audiovisual homepage. Forms can also be submitted in room 510C during the business hours.
Form #9: Schedule Worksheet and Telephone Registration
(only available in the office carousel)
This worksheet is designed to help students plan their daily class schedules. The other side of the page is designed to help ensure that students have all necessary course information (e.g., call number) available when registering by telephone.
Form #10: Registration and Add/Drop Form (PDF)
Please use this form to request a change to your current registration. You may add, drop, or change grading options for courses. Make sure that you have obtained all required signatures before submitting it to Dean Kay.
Form #11: Degree Audit Report: Correction Form (PDF)
Please use this form to request a correction to your Degree Audit Report as it currently appears on Student Services Online (SSOL). This can be used to clarify how a course should fulfill curricular requirements and to fix other errors. Submit the form to Martha McEwan and allow at least two weeks for changes to appear in SSOL.
